The Community Care Service Program (CCSP)
is a waiver program under Medicaid which provides
community-based services to the elderly and those
with physical disabilities. This program allows
members to receive personal care services
in their homes, an alternative to a nursing
home or assisted living.

- Skilled Nursing
- Wound Care
- Light Housekeeping
- Meal Preparation
- Medication Management
- Home-delivered meals
Eligibility
- Must have physical disability (Alzheimer’s and Dementia are valid disabilities)
- Physician approval stating that the individual’s level of care meets the requirements for a nursing home
- Medicaid eligible
- Must agree to received personal services in their home
