waiver program that provides primary
care as well as case management to
people who are physically disabled, frail,
and elderly, also persons with chronic
medical conditions.

- Light Housekeeping
- Skilled Nursing
Eligibility
- Individuals must be Medicaid eligible
- Medicaid approved
- Level of Care must meet nursing home standards
- Medical conditions (diabetes, pulmonary disease, Alzheimer’s)
